- 12th Class Marksheet
- Graduation Marksheet
- Post Graduation Marksheet (only if the candidate has completed post-graduation)
- Valid UPTET (Uttar Pradesh Teacher Eligibility Test) Certificate
- D.El.Ed (Diploma in Elementary Education) Certificate
- Special Note: Candidates who do not have the D.El.Ed certificate at the time of application can upload the relevant D.El.Ed marksheet as a valid substitute document.
- Recent Passport Size Photograph (as per the specified dimension and background guidelines)

Special instructions have been issued specifically for candidates of the 2023 batch: It is recommended to fill the application form only after the 25th of the relevant month, as there is a high probability that the D.El.Ed certificate will be updated on the DigiLocker portal by that time, making it easier for candidates to upload the valid certificate without any issues.
The update also issues a strict warning to all aspirants to be extremely careful while filling the application form, as even a small mistake in the entered details can lead to the rejection of the form at a later stage of the recruitment process. All candidates are requested to cross-verify all the details filled in the application form before final submission.
Aspirants are further advised to keep all the soft copies of the above-mentioned documents organized in a single folder on their device to ensure a seamless and hassle-free application process for the UP PRT 2026 recruitment.
